How It Works

1. Contact Form to be completed by patient.
2. A WMH Doctors Referral Form, or a referral from a family doctor
   Or  Medical Release Form for WMH to forward to family doctor.
3. Copy of Health Card with Photo ID or any government photo ID.

Upon completion of the forms please forward to WMH via fax, email or deliver to WholeMed Health Inc. 28 Roytec Road Unit 5 Woodbridge Ontario L4L 8E4

Scheduling a Doctor’s Appointment

A referral from a family doctor and all the required medical history and treatment information is received, WMH will contact the patient to schedule an appointment with WMH Physician for an assessment.

Appointment Day

WMH doctor reviews the medical history, assesses and examines the patient and determines suitability for medical cannabis treatment. WMH doctor may provide the Medical Document (prescription).

Consultation

Once WMH doctor prescribes medical cannabis, one of our experienced counselors will advise the patient about cannabis use, assist with strain selection and registration with a Health Canada Licensed Producer (LP). Our Staff will provide an explanation regarding the ordering process with the Licensed Producer (LP) and provide any additional information and assistance.
